عوامل الخطر ونتائج الحمل لدى النساء الليبيات المصابات بعدوى المسالك البولية

يُعد التهاب الجهاز البولي (UTI) من المشكلات الطبية الشائعة أثناء الحمل، ويُصنَّف إلى نوعين: غير مصحوب بأعراض أو مصحوب بأعراض. وقد يتعقد بحدوث فشل كلوي، أو تجرثم الدم، أو الصدمة الإنتانية، إضافةً إلى الولادة المبكرة وانخفاض وزن المواليد. هدفت هذه الدراسة إلى تحديد عوامل الخطورة لالتهاب الجهاز البولي أثناء الحمل، وتقدير المضاعفات الأمومية والجنينية المرتبطة به.
كانت الدراسة وصفية مقطعية، شملت 100 امرأة ليبية ولدن في مركز طرابلس الطبي، ولديهن تاريخ إصابة بالتهاب الجهاز البولي خلال عام 2010. جرى مقابلة المشاركات في جناح ما بعد الولادة، والاستفسار عن أعراض التهاب الجهاز البولي أثناء الحمل.
أظهرت النتائج أن 56% من النساء كنّ في الفئة العمرية 26–35 سنة، و65% منهنّ متعدّدات الولادات. أما عوامل الخطورة فتمثلت في: فرط القيء الحملي (59%)، تكرار الإصابة بالتهاب الجهاز البولي أثناء الحمل (36%)، وجود تاريخ سابق للإصابة (69%)، حصى الكلى (5%)، تشوهات خلقية كلوية (2%)، ضعف تناول الماء حيث إن 42% يستهلكن أقل من لتر يومياً، إضافةً إلى مشكلات طبية مثل فقر الدم (62%)، السكري (6%)، وارتفاع ضغط الدم (12%).
أما المضاعفات الأمومية فشملت: الإجهاض (14%)، المخاض المبكر (9%)، تمزق الأغشية قبل المخاض (9%)، تقييد نمو الجنين داخل الرحم (2%)، والإنتان الدموي (1%). بينما المضاعفات الوليدية تضمنت: الولادة المبكرة (9%)، وانخفاض وزن المواليد (7%).
توصي الدراسة بضرورة الفحص المبكر والعلاج الأمثل باستخدام نظام علاجي لمدة 7 أيام في حالات البيلة الجرثومية غير العرضية، وذلك لتقليل المضاعفات الأمومية والجنينية. كما تدعو إلى إجراء المزيد من الدراسات لتحديد عوامل الخطورة والمضاعفات في ليبيا بشكل أدق.
